What is a Log book?
Log book is a day to day record of the clinical and academic work
done by the trainee.
Instructions for filling Log book:
1. Entries must commence from the start i.e. DAY ONE of the training
program.
2. Trainees are advised to make the required entries on the day of
the event and get it signed by the supervisor or any other designated
trainer of
the unit approved for candidate s training duly
identified in the log book.
For the purpose of validating the entries in logbooks a “Consultant”
is an individual who has observed / supervised the procedure documented
by the trainee.
a. For public sector
hospital this individual must be equivalent to the post of senior
registrar or above.
b. For private sector
hospital this individual must possess a major qualification (FCPS
or equivalent) and must be working as a consultant in
the same hospital.
3. It is the responsibility of the trainee to get the logbook signed
by the CPSP approved supervisor as soon as the entries on one page
are completed.
IMPORTANT NOTE :
Trainees are expected to show strict compliance with the instructions
for filling the log books. These instructions have already been duly
notified and reproduced above.
Rules for Submission of Log books:
It has already been informed through Notification No.6-1/Exam- 04/CPS/1438L
dated 21/07/2004 that:
1. Fellowship trainees in subjects in which “Intermediate Module”
has been made compulsory are required to submit two log books to Examination
Department:
a) One log book
for first two years of training along with application form for appearing
in the “Intermediate Module Examination” in March
2005 and onwards.
b) Second log
book for remaining two years or three years (as the case may be for
specialties where training is four years or five years)
along with the application forms for appearing in final Fellowship
examination in September 2007 and onwards.
Log
books of Intermediate Module of Anesthesia, Diagnostic Radiology,
Medicine, Obstetrics/ Gynecology, Ophthalmology,
Otorhinolaryngology,
Pediatrics, Psychiatry and Surgery are available with CPSP.
2. Candidates for Second Fellowship are required to submit log book
for the entire period of specified training along with application
forms for
appearing in Second Fellowship examination.
3. Completed and duly certified logbook will form part of the application
for appearing in the respective examinations i.e Intermediate Module
examination (IM) for year 1 & 2 and FCPS Part
II examination for years 3 & 4 of training.
YEARLY SUMMARY SHEETS
What is a Yearly Summary Sheet?
Yearly Summary Sheets provide a year – wise summary of Case Records
/Procedures/ Operations entered in the log book. The trainee must
fill-in the required columns of the yearly summary sheet for every
patient related entry made in the Section – 1 of the log book in a
chronological order.
Rules for Submission of Yearly Summary Sheets
According to the Notification No.6 – 1/Exam 05/CPS/1545Q dated 28/10/2004
Yearly Summary Sheets should be completed and submitted to the RTMC
at the end of each year of training undertaken since January 2005.
1. It will be the responsibility of candidate to obtain RTMC acknowledgement
of the same.
2. The candidates appearing in September/November 2006 examination
will be required to produce the above acknowledgement alongwith
their application for the examination.
3. Clear photocopies of Yearly Summary sheets for 1st year and 3rd
year (in a four year training program) should be submitted to RTMC,
while
those for 2nd and 4th year will be presented along
with the Log book.
It is reiterated that no log book will be accepted for the
candidates appearing in September/November 2006 examination without
the evidence (RTMC acknowledgement) of prior submission of summary
sheets.
