This offer is for All CPSP Supervisors, Trainees, Fellows*, and Functionaries**.

One device against each CPSP Registration number will be issued.

PTCL Service Order Form will be filled and signed by end user.

Copy of CNIC attached with Form.

Copy of RTMC Certificate / Fellowship Card / Employee ID Card attached with Form.

Device will be issued to end user and will be activated within 48 hrs.

Complaints will be forwarded by individuals through 1218 or 1236 and if delay is observed same will be messaged to PTCL support team.

CPE charges (USB): Rs. 4000/= in four equal installments.

Monthly service charges: Rs.1000/=

This package is only for Doctors who registered with CPSP either Student or Qualified (Not Valid for Doctors who are only MBBS), and Employees of CPSP, documents required are Registration/Fellow Ship Certificate and copy of CNIC..
